Covid Consent Form
The Consent Form can be found at the bottom of this page - please read the following information first.
Important Information :
- The Consent Form must be printed on one piece of paper (i.e page 1 on the front and page 2 on the back). We are unable to accept forms that are printed on more than one piece of paper.
- The Consent Form must be printed in colour, not black and white
- The Consent Form must be returned in an envelope
- The cut off time for returning the form is 9am on Tuesday, 25th January 2022.
- A physical copy must be returned. We are unable to accept email copies.
- The Letter From School provides further details on where the form should be returned to.
Remember - the form must be printed back to back on one page, not on two separate pages.
